If I selected "yes" above, I understand my child must be 8 years old or older and is required to attend the Saturday, Sept 16 (4-5:30pm) audition and my account will be charged a nonrefundable $18 upon submitting this form.

Dancer Agreement
I understand that my dancer will be required to purchase a costume and anyone watching the performance will need to purchase a ticket. Further details to come.
The performances are tentatively scheduled for 2pm and 5pm on Saturday, Dec. 2. I understand both performances are REQUIRED for my dancer, as is the dress rehearsal tentatively at 10am on Saturday, Dec. 2 at Cary Academy. The in-studio dress rehearsal on Saturday, Nov. 18 is highly encouraged.
Placements will be emailed by Tuesday, Sept. 19, and I understand that will have until Thursday, Sept. 21 to decline my role in writing.
If my decline is not received by Thursday, Sept. 21, in writing, I understand that my account will be charged the FULL, nonrefundable, nontransferable tuition on Friday, Sept. 22 — $150 for the 45-minute classes in the session; $140 for the 30-minute classes.
