​Cancellation of Class Form 

    I hereby am requesting the above cancellation of my child's class(es) from Starpath Dance Academy. I realize there is a two-week notice policy prior to the first of the month. Any requests, therefore, made between the 1st and 15th of the month will be considered a cancellation for that month. Any requests made between the 15th and 31st will result in the cancellation of classes the end of the following month.

    I agree that all funds due for this cancellation will be processed from my card on file immediately. Any cancellations after January 31, 2026 will also incur an additional cancellation fee of $75.00.

    ​All monies not paid may result in Starpath Dance Academy forwarding to a outside collection agency.
